Our System Navigators will contact you, complete a Needs Assessment, assist you with getting connected to our services, and possibly even other services in the community depending upon your needs. System Navigators provide needs assessments, referrals, and guidance with accessing community health and social service providers.
How does System Navigation help?
It can be difficult for providers and clients to know all the health and social services that are available in the community. System Navigators help by understanding clients’ service needs, providing education on relevant services, and assisting with referrals and connecting to services as needed. To support clients with language barriers, translation and interpretation services are available to assist with the referral.
Systems Navigators assist people to understand and find their way through health care, and community and social service systems. There are times when people facing life’s challenges do not know what kind of help is available.
